From cranial division of ICA. Supplies initially posteromedial part of cerebral hemispheres. BEFORE these area are finally supplied by caudal division of ICA (becoming PCA)

Type 3 : Increased parieto-occipital supply. Type 4: Increased temporo-parieto-occipital supply2d most common carotid-basilar anastomosis (after trigeminal artery). Usually arises from ICA between C1-C3 levels. Joins the basilar after traversing hypoglossal canal. Extremely rarely arises from the ECA. May be confused with proatlantal artery: but PA passes through the foramen magnum not hypoglossal canal. Usually associated with hypoplasia of the ipsilateral VA and posterior communicating artery. Contralateral VA and posterior communicating artery are also only present in 1/3 of the cases
